How it works
Four steps to your first visit

Call to get scheduled
Call our office at 907-929-4009 to get scheduled as a new patient. We'll start with a brief phone intake to understand your needs and match you with the right clinician.

Complete your paperwork
After your phone intake, we'll direct you to the new-client forms. Complete them digitally or by hand and return them before your first appointment.

Provider-specific consents
Some providers have their own consent form. If you're seeing one of them, you'll complete that consent before your first visit — we'll let you know.

Your first appointment
Come in to our midtown Anchorage office, or connect by telehealth where appropriate, and begin your care.

Your health information is handled under HIPAA. Use the secure portal and our secure upload — never email — for anything sensitive.
Rates & billing
Rates are subject to change, and you have a right to a Good Faith Estimate under the No Surprises Act. Call our office for current rates and billing questions.
