Three steps to refer a patient

    Download the referral form

    Complete the Greatland Clinical referral form with the patient's information and reason for referral.

    Fax it to our office

    Send the completed form to our secure fax at 907-929-4902.

  3. 3

    We'll take it from there

    Our team reviews the referral and contacts the patient to begin a phone intake and scheduling.
